Privacy Policy
This policy explains what information The Phoenix Place PLLC ("we", "us") collects through this website and our mailing list, how we use it, and the choices you have. It covers the website at thephoenixplace.com only.
About your health information. This website and our mailing list are not part of your medical record. Please don't send health details through the website sign-up form or by regular email. If you are a patient, our Notice of Privacy Practices explains how we protect your health information under HIPAA; you receive it when you start care, and you can ask us for a copy at any time.
Information we collect
- When you join our mailing list: your full name, email address and phone number, plus a record of your consent: the wording you agreed to, the date and time, where on the site you signed up, and the version of this policy at that time. We then email you a link to confirm. You are not on our list until you open it, and if you don't confirm within 30 days, we delete your details.
- To prevent spam and abuse: a scrambled, one-way code made from your device's internet (IP) address, and your browser type. We do not store your actual IP address.
- On your own device: a small note in your browser's local storage that remembers whether you closed or completed the sign-up form, so we don't keep showing it to you. It contains no personal details and is never sent to us.
- When you book an appointment: booking happens on our partner's site (Headway), which has its own privacy policy.
We do not use advertising cookies or tracking pixels on this website.
How we use it
- To send you one email asking you to confirm your sign-up.
- To send you the emails you agreed to receive: wellness tips, news about our programs and appointment openings.
- To keep a record that you gave consent, and to honour any request to unsubscribe.
- To protect the sign-up form from spam and misuse.
We do not send marketing text messages. We will only text you for marketing if you separately agree to it in the future.
Who we share it with
We do not sell, rent or trade your information. It is stored on our web hosting provider's secure servers, and only authorised staff of The Phoenix Place can see it. We may share information if the law requires it.
How long we keep it
We keep your details while you are subscribed. If you unsubscribe, we keep your email address and the date you unsubscribed so we can make sure you are not emailed again. You can ask us to delete your details completely at any time.
Your choices
- Unsubscribe: use the unsubscribe link at the bottom of any of our emails, or email us. Unsubscribing takes effect right away.
- See, correct or delete your details: email connect@thephoenixplace.com and we will respond within 30 days.
How we protect it
The website uses encrypted connections (HTTPS). Contact records are stored in a password-protected database that only signed-in staff can access, and every view, export and change is logged.
Children
We provide care to patients ages 10 and older, but our mailing list is for adults, parents and guardians. We do not knowingly collect personal information from children under 13 through this website. If you believe a child has signed up, contact us and we will delete their details.
Changes to this policy
If we change this policy, we will post the new version here with a new version number and date. Each sign-up records which version was in place at the time.
